If your child has asthma and carries an inhaler, the Asthma Action Plan and Medication Authorization and Release Form must be completed and returned to the nurse's office on the first day of school.

If your child has a life-threatening allergy that requires the administration of epinephrine, the parent/guardian and the student's physician must complete the cover letter, Allergy Action Plan and Medication Authorization and Release Form each school year. These forms must be returned to the school nurse's office on the first day of school.

If your child will be taking prescription medication during the school day, the parent/guardian and the student's physician must complete the Medication Authorization and Release Form and return to the nurse's office on the first day the medication starts.

Physicals for all sports are due to the Athletic Director before the first day of practice.

Athletic physicals are valid for 13 months.
